New York City, NY- The February issue of “More” magazine has a useful article about abdominoplasty. It tells the story of a tummy tuck step-by-step, from thinking about it to final healing. The patient accurately describes the pain as quite tolerable.
I have one important point of disagreement with the article. We do almost all tummy tucks as outpatients in our Accredited Surgical Suite. The patient may go home or to a hotel, but always with one of our specially trained registered nurses.

How is a Revision Breast Augmentation Different
From a First Operation?
New York City, NY- The main difference is that the anatomy has been altered. When a woman first has implants, she has normal, undisturbed anatomy and so the operation takes a predictable course.
But if a patient has already had implants, and is not happy, the anatomy is no longer intact, and usually has been changed in an undesirable way. That’s why the patient wants a revision. So a standard approach is out of the question. The surgeon can not just go in and optimistically hope to make things better.
First, I need to understand exactly what you don’t like. Second, I have to identify precisely the anatomical problems that have produced the bad result. And finally, I write down a detailed step-by-step plan to correct the disturbed anatomy.
Revision breast augmentation is not hard if you know in advance what you have to do. No surprises.

Results Last Much Longer with a Lollipop Breast Reduction
New York City, NY – Women are attracted to the lollipop technique to breast reduction (also called Vertical Breast Reduction) because there is much less scarring. But, actually, this operation has an even more important advantage.
The breast tissue itself is sculptured and lifted from the inside. When the breast tissue heals in its new perky position, the result is quite permanent. The skin plays no role in the shape.
Other breast reduction operations rely on cutting out skin and tightening the “skin bra”. The tighter skin will create a perky breast, but only temporarily, because skin stretches and sagging returns in a year.

Tips To Help You Choose A Plastic Surgeon.
1) Is the surgeon a member of the American Society for Aesthetic Plastic Surgery? Members usually do a lot of cosmetic surgery.
2) Did you sense a commitment to excellence in the office. Did the surgeon spend a lot of time with you? Someone who rushes through the consultation may rush through the surgery.
3) Were you treated as an individual? Did the surgeon present you with a surgical plan tailor made for you?
4) Plastic surgeons have to have a good eye and meticulous technique. Did the surgeon show you A LOT of before and after pictures, and did you love the results?
5) Talk to other doctors you know. Established plastic surgeons have a reputation in the community, good or bad.
6) Ask to speak to a patient who has had the procedure you want. You are looking for a surgeon who does a lot of what you want. Many patients are eager to share their experience. Privacy is preserved by having the patient call you.
7) If you are having breast augmentation, ask if the surgeon has a large inventory of different size and shape breast implants available in the operating room. A surgeon who does a lot of breast surgery will have an inventory. This way, the final implant choice does not have to be made in advance.
8) With office surgery, make sure the surgical facility is ACCREDITED. Very important safety assurance.
9) Make sure the anesthesia is given by a BOARD CERTIFIED ANESTHESIOLOGIST. Another very important safety factor.
10) Make sure the office has trained nurses available for hands-on post operative care. This can really speed recovery.

Lifestyle Lift Penalized by New York State Attorney General For Publishing Fake Online Reviews of Its Procedure
New York, NY- We have always been somewhat skeptical about brand name operations since surgical techniques and scientific progress are in the public domain. We have made the point that procedures such as the Lifestyle Lift are basically about marketing.
Now we have evidence that Lifestyle Lift is not just about marketing but about illegal and false marketing. The Lifestyle Lift company has recently agreed to pay $300,000 to settle an accusation brought by New York State Attorney General Andrew Cuomo that Lifestyle Lift engaged in so-called “astroturfing” which is the publishing of fake online reviews of its procedure using employees who pretended to be independent consumers expressing their “satisfaction” with the Lifestyle Lift.
Attorney General Cuomo’s report stated that Lifestyle Lift sent an email to its employees which stated “Friday is going to be a slow day – I need you to devote the day to doing more postings on the web as a satisfied client”.
“This company’s attempt to generate business by duping consumers was cynical, manipulative, and illegal” wrote Attorney General Cuomo.
Certainly, as far as I am concerned, the whole notion of the Lifestyle Lift has lost all credibility.

Facelifts Without the Fuss?
NEW YORK CITY, NY – A potential plastic surgery patient recently inquired about less invasive alternatives to having a facelift. So, what is the best procedure that works like a facelift with minimum fuss?
The answer is simple. It’s important to remember why we do facelifts. Non-invasive procedures will give you a lot of improvement, but you can not get the same results that a facelift would give you. So when considering your options, you have to have realistic expectations.

Should Breast Implants be Replaced every 10 years?
NEW YORK CITY, NY – It is NOT true that breast implants need to be replaced every 10 years. This is a very common idea, but it is not true. If your breast implants look fine and feel fine, leave them alone.
In New York City, we tell breast aumentation patients who have SILICONE implants to have an MRI of the breasts every 2 or 3 years, because a silicone implant could be leaking and you would not know it . The silicone gel stays in place whether the implant shell is intact or not.

It has become a popular term on the internet, and it is short for “Man Boobs”. Man boobs is street talk for gynecomasatia, or breast development in men and boys.
Even though we learned about MOOBS only recently, our plastic surgery practice in Manhattan treats a lot of men with gynecomastia. Dr. Beraka wrote a text-book chapter on gynecomastia a long time ago. And because of this chapter and our big experience, we are referred many men with this problem.
